Design your repair job reports to include defect information for each defect identified in the repair. Using today’s technology, the form would be on a device that wirelessly sends the information to the CMMS. A device with a camera adds richness while reducing typing.
Here are three commonly used defect types:
- Maintenance defect. Maintenance did something wrong. For example, a cover was not properly secured during the last PM and after a few days the cover switch caused the unit to shut down (the switch was an interlock).
- Operations defect. Operations did something wrong. For example, the operator exceeded the allowable limit of motor restarts.
- Design or usage defect. There’s a mismatch between how the unit is designed and how it is used. For example, the motor operates in a high temperature application but its insulation is not rated for that temperature.
The CMMS can generate reports that show similar defects in each area, allowing for root cause corrections. An example is mistake-proofing the motor restart problem with controls that lock out the motor starter when the limit is reached.
