With respect to the equipment listed in the question, it does not matter whether the equipment is cord-and-plug connected or wired directly without cord-and-plug connection; GFCI protection is required for all items listed. See Sections 210.8(D), 422.23, 422.51, and 422.52 of the 2014 NEC.
