Russ found a big hole on the top of this panelboard enclosure.

Russ found a big hole on the top of this panelboard enclosure. This is a very dangerous situation. If someone accidentally drops a tool, bracket, bolt, screw, etc. in this hole, it could generate a short circuit and possibly an arc flash. Section 110.12(A) in the 2020 NEC says that unused openings shall be closed to afford protection substantially equivalent to the wall of the equipment.
